1. What is Knee Osteoarthritis? Learn to identify the symptoms
Osteoarthritis (OA) is a prevalent joint disease primarily affecting middle-aged to elderly individuals, especially after the age of 50. This multifactorial condition arises from a combination of mechanical forces, trauma, inflammation, biochemical reactions, and metabolic disturbances.
According to the World Health Organization (WHO), approximately 528 million people worldwide were living with OA as of 2019, marking a 113% increase since 1990. The knee is the most affected joint, accounting for 365 million cases, followed by the hip and hand. Many individuals experience moderate to severe symptoms that necessitate rehabilitation, and in 2020, around 595 million people aged 30 and older were affected globally. Knee OA is a significant contributor to disability and healthcare expenditures, being the leading cause of knee replacement surgeries.
Key risk factors for osteoarthritis (OA) include genetic predisposition, joint injury, obesity, and being female. Additionally, inflammation and low vitamin D levels have been associated with the progression of the disease.
-
Common symptoms include:
-
Persistent knee pain
-
Stiffness
-
Reduced function
-
Crepitus (in more advanced cases)
-
Restricted joint motion (in more advanced cases)
-
Bone enlargement (in more advanced cases)
-
Synovitis
2. What is Orthocann?
Orthocann is an injectable nanoemulsion for intra-articular administration, combining 25 mg of Cannabichromene (CBC) and 45 mg of Cannabigerol (CBG), two phytocannabinoids with notable pharmacological properties. Furthermore, it adds hyaluronic acid, hydrolyzed collagen, and calcium gluconate, which are essential nutrients that support and promote joint health.
Orthocann was designed for targeted delivery of these bioactive compounds to the application site, offering an innovative option for managing Knee Osteoarthritis.

It works by targeting several molecular pathways in the knee joint, primarily through the endocannabinoid system (ECS), while also using nanoplatform technology to enhance the bioavailability and targeted delivery of its active ingredients, ensuring that they are released gradually into the knee cartilage.
One of the key mechanisms in Orthocann’s action is its modulation of the ECS, specifically through its phytocannabinoid components Cannabichromene (CBC) and Cannabigerol (CBG). These cannabinoids interact with CB2 receptors located in the synovium and cartilage, leading to a reduction in inflammation by inhibiting pro-inflammatory cytokines such as TNF-α and IL-1β. In addition, CBC and CBG modulate neurotransmitter release through CB1 and CB2 receptors in peripheral nerves, which helps alleviate pain and reduce hyperalgesia, providing significant pain relief for OA patients.
Beyond pain relief, Orthocann also contributes to protecting and repairing knee cartilage. Both CBC and CBG inhibit the activity of matrix metalloproteinases (MMPs), enzymes responsible for the breakdown of collagen and proteoglycans in the joint matrix. By inhibiting these MMPs, particularly MMP-3 and MMP-13, Orthocann helps preserve the structural integrity of the cartilage. The inclusion of hyaluronic acid (HA) further enhances joint lubrication and protects chondrocytes, the cells responsible for maintaining cartilage health, by reducing mechanical stress. Hydrolyzed collagen supports the synthesis of type II collagen, promoting tissue regeneration, while calcium gluconate aids in maintaining bone density, reducing the risk of fractures associated with advanced OA.
Orthocann’s nanoplatform technology ensures the efficient delivery and sustained release of its active ingredients into the joint space. This advanced delivery system allows the cannabinoids and other components to penetrate deeply into the synovial fluid and cartilage, maintaining therapeutic levels for longer periods. The slow, controlled release provided by the nanoemulsion enhances the treatment’s efficacy by ensuring that the ingredients target the areas most affected by OA, optimizing symptom management.
Compared to traditional knee injections like corticosteroids, Orthocann offers a safer alternative with fewer side effects. While corticosteroids are effective in reducing inflammation, they are associated with significant risks, such as chondrolysis (cartilage degradation), osteonecrosis (bone tissue death), increased infection risk, and elevated blood sugar levels, particularly with repeated use. In contrast, Orthocann’s blend of natural phytocannabinoids, hyaluronic acid, hydrolyzed collagen, and calcium gluconate not only relieves pain and inflammation but also nourishes and supports joint health, making it a promising long-term option for knee OA treatment without the harmful effects of steroids.
4. Important Information Before Using Orthocann
Consult your doctor if:
-
You are pregnant or breastfeeding, as the safety of CBC, and CBG during these periods is not fully established. Therefore, it is recommended to avoid use in these periods unless indicated by a physician on a risk-benefit basis.
-
You have pre-existing medical conditions, such as liver, kidney, or cardiovascular diseases.
Do not take Orthocann if:
-
If you are allergic to cannabinoids, specifically CBC and CBG, or to Hyaluronic acid, Hydrolysate collagen, or any of the other components of this product.
5. How to use Orthocann
-
Follow your doctor's instructions and the product label for dosage.
-
Prepare the Environment: Ensure a sterile environment for the procedure, including a clean physical space and appropriate personal protective equipment, such as sterile gloves.
-
Select the Needle: Use a sterile 21-Gauge x 1.5-inch needle for the injection.
-
Position the Patient: Position the patient seated or lying supine with the knee flexed at approximately 90 degrees to facilitate access to the intra-articular surface.
-
Choose the Injection Approach: Determine the approach (suprapatellar, lateral, medial, etc.) based on the path of least obstruction and optimal access to the synovial cavity.
-
Conduct a Clinical Assessment: Collect the patient's medical history, confirming the absence of knee swelling or signs of infection.
-
Prepare the Skin: Adhere to strict aseptic protocols for skin preparation, using povidone iodine and isopropyl alcohol 70%. Avoid disinfectants with quaternary ammonium salts.
-
Administer Local Anesthesia: Inject lidocaine 2% for local anesthesia before the intra-articular injection. If ethyl chloride spray is used, cleanse the injection site again.
-
Insert the Needle: Introduce the 21-gauge needle, aspirate the synovial fluid to confirm correct placement, and allow for fluid evacuation.
-
Inject Orthocann: Administer 3 cc of Orthocann smoothly, ensuring there is no resistance or discomfort.
-
Post-Injection Care: Withdraw the needle and discard it properly. Gently move the knee through flexion and extension movements.
-
Patient Instructions: Instruct the patient to flex and bend the knee 3 to 4 times post-injection for better distribution of the injected substance.
-
Apply Dressing: After the injection, apply an adhesive dressing to the site to protect against contaminants.
The recommended dosage in adults is: 1 injection every 3 months for 1 year.
Recommendations:
-
Using an ice pack on the knee for 10-minute intervals, wrapped in a cloth, for the first 24 to 48 hours to alleviate mild pain or swelling.
-
Avoid strenuous activities for 48 to 72 hours post-injection, allowing the joint to rest and recover.
-
Promote Low-Impact Activities to prevent undue pressure on the knee and maintain a healthy weight.
-
Do not discontinue Orthocann without consulting your doctor, even if you begin to feel better. Consistent use is important for maintaining its therapeutic effects.
-
If you have any questions or concerns about how to use this product, consult your doctor.
6. Possible side effects
-
If you experience a skin rash or any allergic reactions, such as itching, swelling, or redness around the injection site, seek medical assistance as soon as possible.
-
